ACADEMIC CONTRACT
The following provisions constitute an agreement between PK-12 Virtual Academy and the Parents / Guardians of a Student / Students.
ENROLLMENT REQUIREMENTS:
1. Student transcripts from the previous school must be submitted.
2. An Immunizations Record / Denial Form (KG - 7th Grade) Picture / Guardian IDs, School Records, Birth Certificate or Student ID must be uploaded on our school website or emailed to info@pk12virtual.com)
3. All students must take a Diagnostics Test to be placed in the appropriate class. This assessment is taken quarterly.
4. Students must have two (2) working devices like a laptop, phone or tablet
5. Driver License or Government ID of the Parent / Guardian.
PROGRAM OPTIONS:
Full-Time Virtual Programs:
Live synchronous instruction with teachers
Students follow a structured academic schedule
Self-Paced Virtual Program:
Independent coursework with no scheduled live instruction
Students’ progress at their own pace using the PKVA learning management system
Combination of in-person and virtual learning
Students must attend two (2) days per week at PKVA learning center
TUITION AND PAYMENT:
6. All students must pay a Non-Refundable Diagnostics Test fee of $200.00.
7. All students must pay a Non-Refundable Registration fee of $200.00.
8. Full fees are applicable if student is tardy, absent or cancels his/her class.
9. No refunds are given for Absences or Holidays.
10. Failure to attend classes does not release the Parent / Guardian from financial obligations.
11. Other fees I may be charged which could include:
A $35.00 charge for any checks returned for non-sufficient funds.
A $50.00 surcharge for any late fees.
12. All tuition is due on the first (1st) of each month, with a grace period of three (3) days. Non receipt of tuition means that students will be inactive in the system.
13. All payments can be made through Credit or Debit Card, Zelle, App (305-798-4866) or Cash. www.pkg-12.com/payments
14. All academic requirements and financial obligations must be completed before the end of the school year.
NON-PAYMENT:
15. If a student finical obligation is not met at an appropriate time PKVA reserves the right to the following:
Students can be suspended from accessing His / Her classes
Students could deny His / Her report card or transcripts
Outstanding balance could be sent to a collection agency
ATTENDANCE AND PARTICIPATION FOR FULLTIME / HYBRID PROGRAM
16. Attendance is critical for graduation.
17. All students must be seen on their computer from neck and above during the entire class session to be considered present.
18. Attendance will be taken in each class, and if the student is not present when attendance is called, they will be marked absent.
19. Any interruption of classes can result in an incomplete grade.
20. All synchronous classes are monitored and recorded by the Administration for future references.
21. All students attending PK-12 Virtual Academy and the Hybrid Learning Environment must attend school for at least 90% of the school year. (180 days)
22. If the student does not attend classes, he/she will be considered in attendance violation. Attendance is 20% of participation grade.
23. The hybrid or full-time students have the option to attend at least twice (2) weekly in our physical location. Open from 10 AM-7PM.
24. Administration will report on the absence of hybrid and full-time students to the Parent or Guardian.
25. Students attending on-line classes must report on time according to their schedule.
26. Parent / Guardian must notify the absence via email (info@pk12virtual) to the school administration.
ACADEMIC:
27. Attend scheduled classes on time.
28 .Complete all coursework and assignments on time.
29. Maintain active participation in classes.
30. Follow PKVA academic policies and procedures.
31 .Failure to complete all course work can result in failing the course.
INCOMPLETE COURSE WORK:
32. Failure to complete any coursework could result in failing the course, incomplete grade and delayed promotion.
33. Any student who fails or completes a course must repeat said course at an additional cost. If the student needs additional assistance, tutoring services are available for an additional cost.
34. If the student accelerates and completes all the requirements to be promoted to the next grade level, a new contract will be required.
35. One on One Tutoring is available upon request for an additional cost.
ACADEMIC INTEGRITY:
36.Students must comply with PKVA’s Academic Integrity Policy. Academic dishonesty includes but is not limited to:
Plagiarism
Cheating
Copying Work
Unauthorized Collaboration
Misuse of Technology During Assessments.
Violations could result in Academic Penalties or Disciplinary Action.
TECHNOLOGY:
37. Students must maintain access to reliable technology
Computer or Laptop. Recommend two devices at all times
Must have reliable internet access
WITHDRAWAL POLICY:
38. Any student who needs to withdraw from PKVA must submit in writing by Parent / Guardian
39. All tuition and fees incurred prior to withdrawal must be satisfied before withdrawal can be processed
40. Withdrawal process takes approximately ten (10) business days to complete
41. Student breaks are outlined in the Parent Handbook and Orientation Handbook.
REPORT CARDS AND TRANSCRIPTS:
42. All report cards and transcripts are issued at the end of the academic year.
43. Students must complete all academic requirements
44. Students must complete all finical obligations
45. PKVA reserves the right to deny any student a report card or transcripts if their obligation is not satisfied.
GENERAL:
46.Students will get their schedules before the first (1st) day of class via email upon receipt of tuition.
47.In case of extenuating circumstances, parents must notify the academy in writing.
48.I understand any changes to this agreement must be negotiated between the Director of PK-12 Virtual Academy and myself at least three (3) months prior to the effective date.
49.I have given my consent for emergency medical care and treatment if the student is physically present at the Academy for any reason.
50.I will keep the school current with all relevant information regarding my child/ren.
51.I have read and will abide by the policies outlined in the student Handbook and terms of this agreement.
FORCE MAJEURE / ONLINE LEARNING DISRUPTIONS:
52.PKVA Shall not be held responsible for interruptions caused by events beyond the school’s control including but not limited to:
Internet Outages
Technology Failures
Natural Disasters
Government Actions
HIGH SCHOOL DIPLOMA:
Number of Courses required for Diploma 24 Courses
Number of Courses transferred X
Required for Graduation
Students will be required to do a minimum of 6 courses per year until completion of the remaining courses for a high school diploma.
No report card will be issued to a student / parent if a student does not complete 3 months at PKVA.
Official Transcript and report card are issued at the end of the school year.
High School Diploma will be issue only when all financial and academic obligations are satisfied.
